INSTRUCTIONS
1. Download and save the file to your computer
2. Fill each form out with the required information
3. You can save your data and re-open the file to modify or enter additional information
4. The Questionnaire/Recommendation Letter (# 12 in the list) should be forwarded and filled out by:
A Priest Your Pastor A Faculty Member Your Actual or Last Employer A Friend
5. Once the forms are complete, please print, sign (if applicable), and send them by email at vocations@sbdiocese.org, or mail them to the Office of Vocations at 12716 Oriole Ave. Grand Terrace, CA 92316
